Behavioral Health Provider - Aledade

Location: West Hills Healthcare
Position: Behavioral Health Provider
Status: 40 hours/week full-time and benefits eligible. Must be available to work all hours the clinic is open as schedule will vary.

Primary Purpose: Work as part of the medical home at West Hills Healthcare Clinic with medical patients to mitigate the behavioral barriers impacting their medical and psychosocial functioning. Behavioral Health Provider will accomplish this by completing an initial assessment, including population-based screeners and brief interventions. They will support the provider team by offering consultation and support on behavioral health issues. This position has a collaborative relationship with the continuum of services in the Yamhill County Care Organization, including the Behavioral Health Provider Learning Collaborative that meets monthly to achieve the standards established by the YCCO.

Education and/or Training:
Must have appropriate credentials and level of training.
Appropriate credentials include:
 Licensed Clinical Psychologist or working towards licensure (post-doctoral candidate)
 Master’s degree in social work and licensed as an LCSW
 Licensed Professional Counselor

Applicant must be licensed in the state of Oregon.

Must have graduate course work in biological basis of behavior, understanding of biopsychosocial model of health, theory, diagnosis, and treatment of mental health and dual diagnosis disorders.
Experience working with people who have experienced chronic medical conditions.
Demonstrate proficiency using DSM V, and ICD-10 and ability to document appropriately in an electronic health record.
Bilingual preferred.

Work Environment:
This job operates in a professional office environment. This role routinely uses standard office equipment such as: computers, phones, photocopiers, and fax machines.

Potential hazards of position:
The worker is exposed to infectious diseases for which immunization is required and provided. The worker is exposed to unpredictable behavior.

Physical Demands: While performing the duties of this job, the employee is regularly required to talk or hear. The employee is frequently required to sit, stand, walk; use hand to finger, and reach with hands and arms. This position requires the ability to occasionally lift office products and supplies weighing up to 5-20 pounds.
